Summary:We report what we believe to be the first case of Toxoplasma pneumonia from sub-Saharan Africa. A 44-year-old man presented to Beatrice Road Infectious Disease Hospital, Harare, with a 2 week history of cough, fever and weight loss. Chest X-ray showed bilateral diffuse shadowing, particularly at the bases. He was treated empirically for pulmonary tuberculosis, but did not respond. HIV-ELISA was positive. Two weeks later fibre-optic bronchoscopy was performed. No intrabronchial lesions were seen. Tachyzoites of Toxoplasma gondii were subsequently identified with Giemsa staining of broncho-alveolar lavage fluid. Unfortunately, the patient's condition deteriorated and he died before the diagnosis was made.
