Summary:The relation between frequent relapse of ulcerative colitis (UC) and rectal cell proliferation at the remission phase was investigated. The 28 UC cases were divided into two groups, short term cases (n =12, less than 5 years) and long term cases (n =16, over 5 years), by the clinical courses. The long term cases were further classifed into two groups, relapse cases (n = 6) and remitted cases (n 10), by the relapse rate. Cell kinetic study was performed, using monoclonal antibody for Bromodeoxyuridine (BrdU) and Ki-67 as the labeling index (LI), and histological activity was assessed. BrdU LI of relapse cases were significantly higher than that of remitted cases (P<0.05). BrdU/Ki-67 ratio of relapse cases were significantly higher than remitted cases and short term cases (P<0.05). The LI of proctitis showed the tendency to be lower than the other types. The LI tended to correlate with histological activity. These results show that rectal cell proliferation and histological activity at the remission phase is useful for early detection of relapse case in the long term UC.
