Summary:A new approach for laparoscopic cholecystectomy (laparoscopic cholecystectomy from fundus downward: LCFD) was investigated. We performed twenty-eight laparoscopic cholecystectomies by the usual technique, and sixty-two were performed by LCFD. The median operating time was 100.7±50.9 min in the laparoscopic cholecystectomy (LC) group versus 77.8±33.0 min in the LCFD group (p<0.01). There was no signficant difference in the median operating time between the LC group and LCFD group of patients without cholecystitis. But in patients with acute cholecystitis, the median operating time was 129.6±25.0 min in the LC group versus 97.4±38.9 min in the LCFD group (p<0.05). Moreover, intraoperative and postoperative complications were fewer in the LCFD group than that in the LC group. LCFD appears to be a safe procedure and does not compromise retrograde laparoscopic cholecystectomy.
