Summary:We report the case of a woman who underwent subcutaneous mastectomy and simultaneous prosthetic reconstruction for malignant disease. Following the procedure, the upper skin flap became progressively ischaemic. A continuous infusion of Iloprost, a prostacyclin analogue, (Schering Health Care Ltd) appeared to salvage the flap. We discuss the possible mechanisms for this action. To our knowledge, this is the first published case in which Iloprost has been used in this way.
