Summary:Septic arthritis is an orthopedic emergency requiring surgical intervention for treatment. We describe the first case of native subtalar septic arthritis at our institution which was managed with open irrigation and debridement using the lateral sinus tarsi approach. The early diagnosis with magnetic resonance imaging (MRI) and immediate intervention allowed reduction of morbidity and preservation of the joint. Subtalar septic arthritis is a rare diagnosis and requires a high index of clinical suspicion for diagnosis. Advanced imaging can help localize source of infection and expedite treatment. Early surgical intervention can help decrease morbidity and preserve joint function.
