Summary:Surgical indications on renovascular hypertension, especially on the patients in chronic stage with low plasma renin activity, have not been settled yet. To solve this problem, the Goldblatt's hypertensive dogs were subjected to successful renal homotransplantation or renal artery reconstruction during both the acute and the chronic phase. This experimental studies demonstrated clearly the antihypertensive effect of normal renal tissue to chronic renovascular hypertension due to other mechanism than renin-angiotensin system. Our clinical experiences and evaluation of collected operative cases in Japan supported this experimental results. Therefore, as far as operative techniques are exellent, renal artery reconstruction is more effective to normalize the blood pressure than nephrectomy, and preoperative measurement of plasma renin activity is very important to he selection of appropriate operation on the patients with renal artery stenosis and hypertension.
