Summary:This observation describes a rare case of development of acute myocardial infarction after orthotopic heart transplantation. The diagnosis of acute coronary syndrome was exposed on the basis of increased cardiac specific enzymes with nonspecific changes in the ECG and an erased clinical picture. When performing coronary angiography, acute thrombotic occlusion of the right coronary artery was revealed. The procedure for coronary angioplasty was complicated by the development of the «no-reflow» syndrome, which was subsequently successfully resolved. The final stage was the stenting of the infarct-conditioned artery with the implantation of two drug-eluting «Resolute Integrity» stents. Later the patient was discharged from the hospital in a satisfactory condition.
