Summary: | A 50-year-old man with silent stones in gallbladder since he was in his 30's, was admitted for postpradial epigastralgia, jaundice and liver dysfunction. He was found to have limy bile into the common bile duct (CBD) which showed a calcific shadow looking like a cholangiography in abdominal X-ray and plain CT. Though the stone was not found in the CBD by endoscopic retrograde cholangiogra-phy, jaundice progressed while he was in the hospital. Cholecystectomy and choledochotomy were carried out for the obstructive jaundice caused by limy bile in gallbladder and CBD with accomponied cholelithiasis. Fine white granular and slimy bile was found in the CBD and over the ampulla of Vater in duodenum, but a stone was not found. White granular lumps adhered to fundus and a fragile stone with similar contents was found in the resected specimen of the gallbladder. Jaundice continued for about one month after the surgery. We report a rare case of obstructive jaundice caused by only limy bile which flowed into the CBD without stones. |
