Summary:A 21-year-old woman was seen at the hospital because of right lower abdominal pain. Abdominal CT scan and US showed a tumor 5cm in longer diameter which was mainly composed of fat in the right lower quadrant of abdomen. A teratoma was suspected. An emergency operation was performed, because a possibility of circulatory disturbance or infection due to torsion could not be ruled out. During surgery, we identified a tumor arisen from the greater omentum outside of the cecum, which was twisted around the omentum. The tumor with part of the greater omentum was resected. No abnormalities were noted in the bilateral ovaries. Histopathological studies revealed corpus albicans and hair inside the tumor and keratinized component in the cystic wall. Thus mature cystic teratoma composed of mature tissues was diagnosed. This paper presents a case of torsion of a mature cystic teratoma of the greater omentum, which is extremely rare.
