Summary:A 25-year-old man diagnosed with uncontrollable perianal abcess was admitted. At first, we strongly suspected of heaving Crohn. s disease, which was eventually. Later, we confirmed Crohn. s disease pahologically from perinial tissue. The perineum was inveterate, and developed into a huge defect. After medication by infliximab, intractable pain under morphine and the defect immediately improved. No case has been reported, to our knowledge, that infliximab being effective for a huge perineal defect and uncontrollable pain in Japan.
