Summary:Complex mycotic aneurysms caused by Campylobacter fetus are rare. The vast majority of previous cases reported are infrarenal. We present a case of pararenal mycotic aneurysm presenting with rupture in a 48-year-old male with AIDS. This was treated with emergent open repair and bilateral renal artery reimplantation, and on 1-year follow-up the patient is doing well with no complications. Open repair for complex ruptured mycotic aortic aneurysms is feasible and should ideally be performed in high-volume tertiary centers with experienced surgeons. Multiple repair modalities exist, as well as options for conduit, and each patient's individual characteristics must be taken into account in order to choose the optimal solution.
