Summary:Concordance for the cancer of the large intestine is extremely rare in identical twins. In the world literature, only a few cases were reported and, the following is believed to be the first case reported in Japan. Case 1. A 32-year-old male was admitted with chief complaints of right lower abdominal pain. Barium enema revealed a filling defect in the caecal region. At laparotomy, a very huge tumor was found which infiltrated into the retroperitoneal space. Ileosigmoidostomy was performed. Pathological examination confirmed the diagnosis of poorly differentiated adenocarcinoma. He died 8 months after operation. Case 2. A 38-year-old male, the identical twin of case 1, was admitted with chief complaints of right lower abdominal pain. Barium enema revealed a filling defect with cuffing sign in the caecum. Ileocaecal resection was performed and the excised specimen revealed an ulcerative tumor measuring 5.5×3.7×0.5 cm. Pathological examination confirmed the diagnosis of well differentiated adenocarcinoma. He died of its recurrence 18 months after operation.
