Summary:We have experienced a case of primary duodenal cancer which is relatively rare. A 46-year-old woman was admitted to the hospital because of upper abdominal pain, of which ailing time was very short and no jaundice was associated. Imaging methods revealed a whole circumferential tumor with vacant lumen in the descending part of duodenum. Biopsy revealed tubular adenocarcinoma. A pancreatoduodenectomy was performed and the patient's postoperative course was uneventful. The resected tumor was 8.8×9.3cm on the resected fresh specimen or 8.6×5.7cm on the fixed specimen, which is the largest in the Japanese literature. Pathologically it was moderately differentiated tubular adenocarcinoma which infiltrated into the pancreas but did not involved the bile duct, papilla of Vater, pyloric ring, and regional lymph nodes. The patient has been followed periodically. Thyroid cancer was found 3 years 2 months after the operation. Abdominal imaging scanning performed at thyroidectomy 3 years 7 months after the operation revealed no findings of recurrence. The patient has been free from recurrence as of 4 years 6 months after the pancreatoduodenectomy.
