Summary:We report a case of gastric cancer producing both granulocyte-colony stimulating factor (G-CSF) and alpha-fetoprotein (AFP). A 76-year-old man who presented with a main complaint of dizziness and general fatigue was admitted to our hospital with severe anemia. Upper gastrointestinal endoscopic examination revealed type 2 gastric cancer on the antrum-greater curvature of the stomach. The pre-operative laboratory data indicated leukocytosis (33,150/μl), a high level of serum G-CSF (659 pg/ml) and AFP (171.2 ng/ml). The patient underwent distal gastrectomy with D1+ lymph node dissection and partial resection of the transverse mesocolon. Pathological findings were papillary adenocarcinoma, pT3 (SS), med, INFa, ly1, v0, pN2, P0, CY0, M0, Stage IIIA. Immunohistochemistry staining with anti-G-CSF and anti-AFP antibody was positive in the tumor cells. Postoperatively, the leucocyte count decreased to 6,530/μl, serum G-CSF to 42.7 pg/ml and serum AFP to 37.9 ng/ml. To the best of our knowledge, this is the first case report of gastric cancer producing both G-CSF and AFP in Japan.
