Summary:A 39-year-old woman noted a nodule at the right lower anterior neck. Fine-needle aspiration cytology showed squamous cell carcinoma. We performed surgical resection of the tumor. On immunohistochemical examination, tumor cells stained positively for CD5, CD56, CD117, CEA, c-kit, CK7, chromogranin A, and synaptophysin. We diagnosed her with carcinoma showing thymus-like differentiation (CASTLE). CASTLE is a rare malignancy that occurs in the soft tissues of the neck or thyroid gland. Complete resection is important to improve the long-term survival and locoregional recurrence rates.
