Summary: | Objective To explore the application effect of the Toyota production system (TPS) in optimizing the process of one-stop hospital admission and discharge service. Methods A total of 1, 140 patients from three campuses of a Grade-A hospitals between March 2023 and December 2024 were enrolled as research subjects. Among them, 566 patients admitted before TPS optimization (March 2023 to January 2024) were assigned to the control group, while 574 patients admitted after TPS optimization (February 2024 to December 2024) were assigned to the observation group. Based on the core principles of TPS, the value stream of admission and discharge processes was analyzed, non-value-added steps were identified through value stream mapping, and process reengineering, resource integration, and standardized workflows were implemented. The admission/discharge processing time and patient satisfaction were compared between the two groups. Results The average admission/discharge processing time was reduced to 18. 50 min in the observation group compared to the 199. 00 min in the control group; The number of process steps decreased from 15 to 8, with value efficiency increasing to 37. 50% and process efficiency rising to 37. 84%. Patient satisfaction in the observation group was higher than that in the control group (91. 46% vs. 66. 08%, P<0. 05). Conclusion The implementation of TPS optimization simplified the admission/discharge workflow, reduced processing time, enhanced patient satisfaction, alleviated the pressure on manual service counters, and improved the quality of medical and nursing services. (目的 探讨丰田生产方式(TPS)在优化“一站式”入出院服务流程中的应用效果。方法 选取2023年3月—2024年12月某三甲医院及其分院、附属医院三个院区的1140例患者作为研究对象, 其中2023年3月—2024年1月实施TPS优化前办理入出院的566例患者作为对照组, 2024年2月—12月实施TPS优化后办理入出院的574例患者作为观察组。基于TPS核心原则, 分析入出院价值流程, 绘制流程价值流图, 识别非增值环节, 进行流程重构和资源整合, 建立入出院标准化作业。比较两组的入出院办理时间和患者满意度。结果 实施TPS管理优化“一站式”入出院办理流程, 入院出平均办理耗时从199. 00 min缩减为18. 50 min; 流程步骤由15项精简至8项, 价值效率由20. 00%提升至37. 50%, 流程效率由4. 52%提升至37. 84%。观察组患者满意度(91. 46%)高于对照组(66. 08%), 差异有统计学意义(P<0. 05)。结论 TPS能够简化患者入出院办理流程, 缩减办理时间, 提高满意度, 缓解了人工窗口压力, 对提高医疗护理服务质量具有积极意义。) |
