Summary:A 54-year-old man visited our hospital with the chief complaint of right lower quadrant abdominal pain. An abdominal computed tomography (CT) scan revealed multiple diverticula in the appendix and an increase in the surrounding fat concentration. Based on these findings, the patient was diagnosed as having appendiceal diverticulitis. A laparoscopic appendectomy was thus performed. The histopathological studies revealed multiple true diverticula with thinning muscular layer in the appendix, one of which showed inflammation. Finally true appendiceal diverticulitis was diagnosed. Appendiceal diverticulitis is reported to be associated with perforation in a high frequency, and to be extremely difficult to diagnose preoperatively. It is often diagnosed after surgery with the diagnosis of acute appendicitis. Almost all diverticula of the appendix are pseudodiverticula and true diverticulum of the appendix is rare. We report a case of true appendiceal diverticulitis.
