Summary:The purpose of this case report was to describe the long-term follow-up of the space closing of maxillary incisors with advanced periodontitis. A 47-year-old female with a noncontributory medical history was referred to our hospital with a chief complaint of pathological migration of both maxillary central incisors. A complete oral examination revealed the pronounced loss of attachment apparatus in most parts of the dentition. In intial treatment, the space closing was pursued by simple fixed minor tooth movement appliance with elastic materials. Following the space closing, the anterior teeth were treated by a flap operation to remove deep periodontal pockets. Teeth were then cemented with a maxillary lingual adhesion metal plate. During the first year after completion of the active therapy the patient was enroled in a maintenace care program with monthly appointments and thereafter once every 3 to 6 months. Her periodontal condition such as probing depth and alveolar bone has not deteriorated in the last 13 years. As shown in this case, pathologic migration of the maxillary anterior teeth can be improved in patients with reduced periodontal support, and offer a better long-term prognosis together with good plaque control.
