Transarterial embolization for thoracolumbar spinal dural arteriovenous fistula fed by the same segmental artery as the artery of Adamkiewicz: a case report

Summary:Objective: We report a case of transarterial embolization for spinal dural arteriovenous fistula (SDAVF) fed by the same segmental artery as the artery of Adamkiewicz (AKA). Case presentation: A 74-year-old female presented with lower limb weakness, sensory disturbance, and sphincter dysfunction. An MRI showed spinal cord edema with perimedullary flow voids at thoracolumbar segment. Spinal angiography revealed SDAVF fed by the left first lumbar artery, which also contributed to AKA. Superselective and sufficient length of catheterization into the feeder enabled transarterial embolization using n-butyl-2-cyanoacrylate, and the fistula was completely obliterated with preservation of AKA. Conclusion: Embolization of SDAVF fed by the same segmental artery as AKA may be possible under limited anatomical conditions.
