To report a case of a spontaneous resolution of a macular hole resulting from injury.
A 17-year-old man experienced a blunt injury: fundus examination found a full-thickness macular hole. It was studied with a clinical exam and optical coherence tomography images. After two weeks, the macular hole spontaneously disappeared.
Blunt injury is the most frequent ocular injury. The consequences can be very serious because of possible lesions in the macular area: the visual function prognosis can therefore be disastrous. Macular holes can have a good prognosis, and spontaneous closure is sometimes observed. This report attempts to explain the mechanisms.
Disappearance of a macular hole resulting from injury can occur spontaneously, especially in young patients. OCT is today indispensable, making it possible to follow up the injury and its consequences objectively, reliably and with very good reproducibility.
Présentation d’un cas de trou maculaire post-traumatique spontanément résolutif.
Un jeune homme, âgé de 17 ans, présenta dans les suites d’une contusion oculaire un trou maculaire, de petite taille. Il fut décidé de réaliser une surveillance clinique rapprochée par l’examen clinique habituel et par l’OCT-3. On observa alors une guérison spontanée de ce trou maculaire en deux semaines.
Les contusions sont les traumatismes oculaires les plus courants. Leur impact sur le pôle postérieur de la rétine peut avoir de graves conséquences sur le devenir fonctionnel visuel. Les trous maculaires restent, parmi ces atteintes maculaires, des lésions de pronostic possiblement bon et dont la résolution peut se faire spontanément. Certaines hypothèses tentent d’en expliquer les mécanismes.
La fermeture spontanée d’un trou maculaire post-traumatique n’est pas rare, d’autant qu’il survient chez un patient jeune. L’OCT est indispensable, permettant désormais un suivi objectif, fiable et reproductible. |
