Summary:A bone marrow biopsy sample showed normocellularity and multifocal aggregates of abnormal spindle shaped mast cells (figs 1 , 2 , and 3 ). Indolent systemic mastocytosis is distinguished from illnesses with similar clinical manifestations principally by the presence of raised serum tryptase concentrations and histological features, although these criteria are not always present in monoclonal mast cell activation syndrome.
