Summary: | Heart rate was 105 beats/min; blood pressure was 135/95 mm Hg; respiratory rate was 20 breaths/min; and oxygen saturation was 97% on ambient air. Cardiac MRI Answer: B Electrical alternans, a characteristic alternating QRS amplitude reflecting the swinging of the heart in the pericardial space, is specific (>85%) but not sensitive (estimates ≤25%) for the diagnosis of large pericardial effusion (see online supplemental video).1–3 A large, globular cardiac silhouette (‘water bottle’ sign) on chest radiograph is also suggestive (figure 2). Role of a 12-lead electrocardiogram in the diagnosis of cardiac tamponade as diagnosed by transthoracic echocardiography in patients with malignant pericardial effusion. |
