Cardiac MRI Answer: B Electrical alternans, a characteristic alternating QRS amplitude reflecting the swinging of the heart in the pericardial space, is specific (>85%) but not sensitive (estimates ≤25%) for the diagnosis of large pericardial effusion (see online supplemental video).1–3 A large, globular cardiac silhouette (‘water bottle’ sign) on chest radiograph is also suggestive (figure 2).
