치료받지 않은 고혈압 환자에서 발생한신동맥류의 자발성 파열

비외상성 신동맥류는 주로 섬유근성 증식증 혹은 동맥경화증 등의 혈관 질환이나 결절성 다발성 동맥염, 임신 등과 관련되어 나타난다1-4). 비외상성 신동맥류는 대개 별다른 증세를 유발하지 않아 과거에는 매우 드문 질환으로 인식되었으나 최근에는 임상에서 복부 전산화 단층촬영, 신동맥 조영술의 방사선학적인 검사 빈도가 많아지면서 점차 증가되고 있는 추세이다1-4). 대부분의 신동맥류 환자에서 고혈압이 동반되지만 신동맥류가 고혈압을 야기하는지 혹은 이 두 질환이 단순히 동반되는지에 대해서는 현재까지 논란이 되고 있다. Spontaneous rupture of renal artery aneurysm is a rare, but life threatening complication with high mortality. The etiology of non-traumatic renal artery aneurysm is fibromuscular dysplasia, atherosclerosis, vasculitis, and pregnancy. We here report a case of spontaneous rupture of renal artery aneurysm in a patient with untreated hypertension. A 39-year-old non-pregnant woman complained of sudden onset of right flank and lower quadrant abdominal pain. Ultrasonography showed large fluid collection in right lower abdomen. Emergency laparotomy demonstrated huge retroperitoneal hematoma due to spontaneous rupture of right renal artery aneurysm. Microscopic examination of the aneurysmal wall revealed intimal hyperplasia without atherosclerotic change. KCI Citation Count: 2
