Disclosed are a geothermal pipe used to exhaust heat inside a building to the ground to cool the building, or to obtain heat required to heat the building from heat on the ground which is from geothermal heat; and an apparatus and a method to manufacture the same. The geothermal pipe comprises: a pipe-shaped main body extended in a longitudinal direction; and a heat transfer rib spirally protruding in the inner circumferential surface of the pipe-shaped main body. Moreover, the apparatus to manufacture the geothermal pipe comprises: an extruder to melt and supply synthetic resin; a die on which a mandrel is mounted having a plurality of grooves on a surface in a longitudinal direction, and to form the pipe-shaped main body having the heat transfer rib on a straight line in the inner circumferential surface by distributing molten synthetic resin supplied from the extruder in the pipe form through a flow path where the molten synthetic resin moves between the mandrel and the die; and a rotational device mounted on the outer circumferential surface of the pipe-shaped main body, and to transform the heat transfer rib on a straight line into a spiral heat transfer rib by rotating and twisting the pipe-shaped main body.
