To provide a positioning and stabilizing structure for holding a seal forming structure at a therapeutically effective position on the head of a patient.SOLUTION: The seal forming structure is constructed and arranged, in use, to form a seal with a region of a patient's face enclosing the entrance to the patient's airways so as to deliver, in a sealed manner, the flow of air by predetermined therapeutic pressure against ambient air pressure in an overall respiratory cycle of the patient. The positioning and stabilizing structure may comprise at least one gas delivery tube to deliver the flow of air to the entrance of a patient's airways via the seal forming structure. The at least one gas delivery tube may be constructed and arranged to contact, in use, at least a region of the patient's head above the otobasion superior of the patient's head.
