Abstract | Noninvasive imaging in the assessment and prevention of coronary heart disease RESUMEN La ecocardiografía, la tomografía computarizada multidetectores, la resonancia magnética y la cardiología nuclear contribuyen a prevenir la enfermedad coronaria. La ecocardiografía, con el estudio del grosor íntima-media carotídeo, la rigidez arterial y la dilatación mediada por flujo en la arteria braquial, identifica a individuos asintomáticos en riesgo de enfermedad coronaria tributarios de terapias preventivas agresivas. El score de calcio es un predictor independiente de eventos cardiacos y modifica scores de riesgo clínico como el de Framingham. La coronariografía por tomografía multidetectores y la resonancia magnética visualizan la pared y la luz arterial y detectan placas con y sin calcificaciones antes que la coronariografía invasiva. La cardiología nuclear permite la evaluación de la repercusión funcional de la lesión aterosclerótica mediante el análisis combinado de la perfusión y la función ventricular. ABSTRACT Echocardiography, multidetector computed tomography, magnetic resonance imaging, and nuclear cardiology can all help prevent coronary heart disease. Echocardiography can identify asymptomatic individuals who are at risk of coronary disease and who should receive aggressive preventative therapy by providing data on the carotid intima-media thickness, arterial stiffness and flow-mediated dilatation of the brachial artery. The calcium score is an independent predictor of cardiac events that influences clinical risk scores such as the Framingham risk score. By using multidetector computed tomography or magnetic resonance imaging for coronary angiography, it is possible to visualize both the lumen and vessel walls of coronary arteries and to discriminate between calcified and noncalcified atherosclerotic plaque before invasive coronary angiography is performed. With nuclear cardiology, the functional effects of atherosclerotic lesions can be evaluated by assessing perfusion and ventricular function simultaneously.
