Yatarak tedavi gören erkek alkol bağımlısı hastalarda alkol aşermesinin travma sonrası stres bozukluğu ve genel psikopatolojinin şiddeti ile ilişkisi

Amaç: Bu araştırmanın amacı, yatarak tedavi gören alkol bağımlılarında alkol aşermesinin travma sonrası stres bozukluğu (TSSB) ve genel psikopatolojinin şiddeti ile ilişkisinin saptanmasıdır. Yöntem: Bakırköy Ruh Sağlığı ve Sinir Hastalıkları Eğitim ve Araştırma Hastanesi, Alkol Madde Araştırma, Tedavi ve Eğitim Merkezi'ne (AMATEM) Temmuz 2007-Şubat 2008 tarihleri arasında yatarak tedavi gören hastalar çalışma kapsamına alındı. DSM-IV tanı ölçütlerine göre alkol bağımlılığı tanısı konan 103 hasta çalışma örneklemini oluşturdu. Hastalar sosyodemografik soru formu yanısıra, Klinisyen Tarafından Uygulanan Travma Sonrası Stres Bozukluğu Ölçeği (TSSB-Ö), PENN Alkol Aşerme Ölçeği (PAAÖ), Belirti Tarama Ölçeği (SCL-90) ve Michigan Alkolizm Tarama Testi (MATT) ile değerlendirildi. Bulgular: PAAÖ toplam puanı SCL-90 genel belirti indeksi ve 9 alt ölçekten somatik belirtiler ve obsesif belirtilerle ileri derecede pozitif korelasyon gösterirken diğer alt ölçeklerde orta düzeyde korelasyon ve MATT ile zayıf düzeyde korelasyon göstermiştir. TSSB olanlarda (%32) aşerme toplam ve madde puanları 4. madde (direnme) dışında yüksek bulunmuştur. Aşermeyi belirleyen değişkenleri saptamak için yapılan linear regresyon ve TSSB'nun sabit değişken olarak alındığı univariat kovaryans analizinde aşerme puanını belirleyen tek değişken obsesif belirtilerin şiddeti olmuştur. Alkol aşermesi TSSB ve genel psikopatoloji ile ilişkili görünürken aşermenin psikopatoloji açısından tek belirleyicisi obsesif belirtilerin şiddeti olmuştur. Çalışmamızın bulguları, aşermenin obsesif kompulsif belirtilerle örtüştüğü genel görüşünü destekler niteliktedir Objective: In this study,the relationship of alcohol craving with posttraumatic stress disorder and severity of general psychopathology in male alcohol dependent inpatients were determined. Method: The study was conducted with hospitalized patients between July 2007-February 2008 in Bakirkoy State Hospital for Mental Health and Neurological Disorders, Alcohol and Drug Research, Treatment and Education Center (AMATEM) in Istanbul. One hundred and three inpatients diagnosed as having alcohol dependence were included in the study. They were given a sociodemographic form, Clinician Administered Post Traumatic Stress Disorder Scale (CAPS), PENN Alcohol Craving Scale (PACS), the Symptom Checklist-Revised (SCL-90) and Michigan Alcohol Screening Test (MAST). Results: The PACS total score showed high positive correlations with SCL-90 general symptom index and somatization and obsession subscales of SCL-90, whereas it was modestly correlated with the other subscales of SCL-90 and mildly correlated with MAST. Total craving score and each item other than the 4th item (resistance) were higher among those with lifetime PTSD. The Obsession subscale was the only predictor of craving score in the linear regression model and univariate analyses, when PTSD was taken as the fixed variable. Discussion: Although alcohol craving was related with TSSB and general psychopathology, the only predictor of craving was the severity of obsessive symptoms among the other psychopathologies. This finding supports the general view that craving is overlapped with obsessive compulsive symptoms
