Summary:The purpose of this article is to provide an overview of the literature on factors associated with hospital readmission of older heart failure patients. Important factors reported to be related to rehospitalization are sociodemographic and medical factors, premature discharge, failing support system, medication-related problems and noncompliance. To prevent readmission, interventions in the area of discharge planning, patient education and follow-up are recommended.
