안전한 이종이식 연구를 위한 식약처 가이드라인 개정(안) 연구

최근 미국의 이종이식 사례는 발전한 유전자 편집 기술을 활용한 유전자 편집 돼지의 장기를 살아있는 사람에게 시행한 임상시험이라는 점에서 주목받고 있다. 국내에서는 2019년 「첨단재생 의료 및 첨단바이오의약품 안전 및 지원에 관한 법률(이하, ‘첨단재생의료법’)」이 제정되면서 이 종이식 임상연구에 관련된 법적 환경이 변화되었다. 이에 따라 연구자들이 이종이식 연구를 진 행할 때 비임상시험부터 임상시험, 상용화에 이르기까지 수행할 수 있도록 안내할 기준이 필요 하다. 식약처는 2006년도에 세 가지 가이드라인을 통해 원료동물, 품질과 감염관리, 전임상과 임 상에 관한 사항을 연구자들에게 안내하고 있었는데, 기술 개발이 안정화되고 국내·외 규제 현황 이 변화함에 따라 이의 개정 요구가 있었다. 본 연구진은 국내·외 규제 현황을 조사하고 이를 토 대로 가이드라인 개선 방안을 다음과 같이 제안한다. 구조적인 면에서 현행 가이드라인이 원료동 물, 품질과 감염관리, 전임상과 임상시험의 세 가지 항목으로 분류한 것에서, 원료동물, 품질과 감염관리, 비임상시험, 임상시험의 네 가지 항목으로 구분하고 분류하여 임상시험에 관한 사항 을 구체화하고자 하였다. 그리고 개선 방안으로 첫째로는 용어와 정의를 수정하고 변경하여 개념 을 명료화하고자 하였다. 둘째로는 감염원 목록을 개정하여 감염관리를 강화하고자 하였고, 셋째 로는 동물대상 연구의 동물 윤리 조항을 신설하거나 강화하였다. 넷째로는 환자의 안전과 인권을 고려한 임상시험의 안정성을 확보하고자 하였고, 마지막으로 연구자의 윤리적, 사회적 책임을 강 화할 수 있도록 연구자를 위한 선언적인 원리를 새롭게 마련하여 담을 수 있도록 하였다. 결과적 으로 이종이식 연구자들이 원활하게 연구할 수 있고, 환자들이 안전하게 연구에 참여할 수 있도 록 가이드라인을 정비하고 이종이식이 장기이식의 대체재로서 역할을 할 수 있기를 바란다. The recent case of xenotransplantation in the U.S. drew attention since it was a clinical trial of a gene-edited pig organ in a living person using advanced gene editing technology. It is anticipated that xenotransplantation could replace homologous organ transplantation in the near future. The researchers investigate the current status of do- mestic and international regulations and propose measures to improve guidelines based on them as follows. In terms of structure, the current guidelines are classified into three categories: animal donor, quality and infection control, preclinical and clinical trials, and tried to specify clinical trials by classifying them into four categories: animal donor, quality and infection control, nonclinical tests, and clinical trials. As an revision plan, the first step was to clarify the concept by modifying and changing the terms and definitions. Second, the list of infection sources was revised to strengthen infection control. Third, animal ethics provisions for animal research were newly established or strengthened. Fourth, it was intended to secure the stability of clinical trials considering the safety and human rights of patients. Finally, in order to strengthen the ethical and social responsibil- ity of researchers, a new declarative principle for researchers was prepared and contained. In conclusion, it is hoped that xenotransplantation researchers can study smoothly, reorganize guidelines so that patients can participate in the study safely, and that xeno- transplantation can serve as an alternative to organ transplantation.
