기억성 경도인지장애 환자의 언어수행력

배경 및 목적: 경도인지장애 (mild cognitive impairments, MCI)는 본인의 생활연령 및 교육년수와 비교하여 인지기능의 감퇴가 좀 더 심한 사람들을 일컫는 용어로서 이들의 일상생활기능은 정상적인 것으로 보고된다. 본 연구는 정상노인과 기억성 경도인지장애 (amnestic mild cognitive impairments, aMCI) 환자의 변별을 위한 언어과제의 유용성을 확인하고, 두 집단 간 수행력을 비교함으로써 최적의 절단점 (optimal cut-off score) 산정을 위한 참조적 정보를 파악하고자 하였다. 방법: 정상노인 23명, aMCI 환자 33명을 대상으로 대면이름대기, 생성이름대기 (의미유창성, 음소유창성 과제), 문장이해력, 그림설명하기(분석 변인: 초당 음절수, CIU 비율(%), 대명사 산출비율(%)) 과제에서의 수행을 비교, 분석하였다. 결과: 그림설명하기 과제의 초당 음절수와 대명사 산출비율을 제외한 나머지 언어과제에서 aMCI 환자 집단이 정상노인 집단에 비해 유의하게 낮은 수행을 보였다. ROC 분석 결과, 문장이해력, 한국판 보스톤 이름대기 검사 단축형 (K-BNT-15), CIU 비율(%) 순으로 두 군에 대한 과제별 변별도가 높게 나타났으며, 민감도 범위는 63.1∼87.7%, 특이도 범위는 65.1∼97%였다. 논의 및 결론: 위 결과는 여러 가지 언어과제에 대한 정상노인과 aMCI 환자의 수행력 비교를 통해 이러한 과제들이 aMCI 환자의 조기 감별에 유용하게 사용될 수 있음을 시사하고 있다. 치매로 이환될 가능성이 높은 MCI 환자의 조기 발견은 치매로의 이행속도를 늦추는 또 다른 치료 선택사항이 될 수 있으며, 이를 위해 본 연구 결과가 유용하게 활용될 수 있을 것이다. Purpose: Mild cognitive impairment (MCI) is a syndrome defined as cognitive decline greater than expected for an individual's age and level of education, but that does not interfere notably with activities of daily life. The purpose of this study was to estimate the optimal cut-off scores by comparing performance according to language tasks for the discrimination between normal elderly and patients with amnestic mild cognitive impairment (aMCI). Methods: The performance of 23 normal elderly and 33 patients with aMCI in tasks such as confrontation naming, generation naming (semantic fluency, phonemic fluency), sentence comprehension, picture description (number of syllables per second, CIU ratio (%), pronoun production ratio), and targeting was measured. Results: There was a significant difference in all language tasks except for the number of syllables per second (speech rate) and pronoun production ratio. In ROC analysis, the language task showing the greatest discrimination between the control group and the aMCI group was 'sentence comprehension', 'K-BNT-15', 'CIU ratio (%)', 'letter fluency task (/ㄱ/)', and 'semantic fluency task (animal)'. Conclusions: The present study suggests that deficits in sentence comprehension, naming, and connected speech are identifiable in patients with aMCI. Therefore, these language tasks are suitable for detecting subtle language deficits in subjects with aMCI. Specific language screening tasks, i.e., sentence comprehension, naming, CIU ratio (%), were found to be useful to clinicians for assessing the degree of cognitive impairment. Increasing attention is being paid to persons with mild cognitive impairment for whom treatment options are being evaluated that may alter the rate of progression to dementia. KCI Citation Count: 2
