항바이러스제 정맥주사 및 유리체내 주입술을 병합하여 치료한 급성망막괴사 2예

목적: 정상 면역을 가진 환자에서 발생한 급성망막괴사를 항바이러스제 정맥주사 및 유리체내 주입술을 병합하여 효과적으로 치료한 2예를 보고하고자 한다. 증례요약: 급성망막괴사로 진단받은 정상 면역 상태인 환자 2명(2안)에게 acyclovir 정맥 내 주사(1,500 mg/m2/day)를 시행하였으나 치료에 반응하지 않고 2명 모두 망막괴사가 진행하여 2~3회의 gancyclovir (2,000 μg/0.05 ml) 유리체내 주입술을 실시하였다. 이후 2안 모두 망막괴사는 호전되고 병변 또한 진행하지 않았으며, 1년 6개월의 관찰 기간 동안 안정된 상태를 유지하였다. 결론: 유리체내 항바이러스제 주입술은 고전적인 치료에 반응하지 않는 급성망막괴사 환자에서 비교적 안전하고 효과적으로 사용할 수 있는 보조 치료법으로 고려할 수 있다. <대한안과학회지 2009;50(5):794-799> Purpose: To report the use of intravitreal antiviral injections as adjunctive therapy in the managementof two immunocompetent patients with acute retinal necrosis. Case summary: We performed two or three intravitreal injections of gancyclovir (2,000 μg/0.05 ml) on two patients (two eyes) with acute retinal necrosis resistant to intravenous acyclovir therapy (1,500 mg/m2/day). Both patients received intravitreal antiviral injections for the treatment of retinitis that progressed despite standard intravenous acyclovir therapy. The retinitis resolved, and visual acuity improved after 18 months of follow-up in both cases. Conclusions: Intravitreal antiviral injections may be a safe and efficacious adjunctive therapy in the management of patients with acute retinal necrosis resistant to intravenous acyclovir therapy.
