Abstract | 목적: HTG-1 (Huvitz, Anyang, Korea)과 Pentacam HR (Oculus, Wetzlar, Germany)의 측정값을 국제규격인 ISO19980:2012에서 지정된 방식으로 분석하여 HTG-1의 임상적 유효성을 입증하고자 하였다.
대상과 방법: 75명의 75안을 ISO19980:2012에서 지정한 각막 곡률 측정의 영역에 대해 대조기기인 Pentacam HR과 시험용기기인HTG-1로 각막을 측정하였다. 이후 측정값인 각막 곡률 및 난시, 각막의 비구면성, 각막의 크기 등에 대해 두 기기 간 비교 분석을시행하였다. 대상들은 각막 난시 기준으로 1.5 D 미만은 그룹 1, 1.5 D 이상은 그룹 2로 분류하여 그룹별 분석을 시행하였다.
결과: 총 75안 중, 그룹 1에는 38안, 그룹 2에는 37안이 포함되었다. 두 장비에서 측정된 값에 대한 상관성을 분석한 결과 HTG-1과Pentacam HR 간 모든 종류의 각막곡률값과 각막 난시 값이 유의한 차이를 나타내었다(p < 0.001). Bland-Altman plot에서의 95% limits of agreement는 모든 측정 zone에 대해 모든 측정치에 대해 대체로 repeatability limit 내에 존재하여 일치도가 높았다.
결론: HTG-1과 Pentacam HR은 두 기기 간의 측정치의 반복성과 일치도는 높은 것으로 확인되었다. 일부 값, 특히 flat K값의 일치도에 대한 영점 조절을 한다면 Pentacam HR의 측정치를 대체할 수 있을 것으로 보인다. Purpose: This study compared the keratometry values obtained from the HTG-1 (Huvitz, Anyang, Korea) and the Scheimpflug-based Pentacam HR (Oculus, Wetzlar, Germany) in accordance with ISO19980:2012. The study aimed to assess the repeatability, reproducibility, and agreement between the devices, thereby evaluating the clinical validity of the HTG-1 as a corneal topographer.
Methods: A total of 75 eyes from 75 subjects were examined for corneal curvature measurements following the ISO19980:2012 standards. The measurements were taken in the central (3 mm zone), middle (5 mm zone), outer (7 mm zone), and simulated K areas. These included steep K, flat K, mean K, astigmatism, and axis, as measured by both the HTG-1 and Pentacam HR.
Astigmatism and axis were further analyzed using Jackson Cross Cylinder values (J0 and J45) through power vector transformation. Additional parameters such as corneal asphericity, white-to-white measurements, Keratoconus index, and Keratoconus prediction index were also compared. To investigate how astigmatism might influence these comparisons, subjects were divided into two groups: Group 1 with less than 1.5 D of astigmatism, and Group 2 with greater than or equal to 1.5 D, followed by subgroup analysis.
Results: In Group 1 (n = 38) and Group 2 (n = 37), correlation analysis revealed statistically significant differences (p < 0.001) between HTG-1 and Pentacam HR for all corneal curvature and astigmatism measurements. However, the 95% limits of agreement from the Bland-Altman plots generally fell within the repeatability limits for all measurement zones, demonstrating a high degree of agreement between the two devices.
Conclusions: Despite employing different measurement principles, the HTG-1 and Pentacam HR exhibit a strong repeatability and agreement across various keratometry values.
