Abstract | Objectives In this study, we identified the symptoms of insomnia, anxiety, and depressed mood in newly diagnosed women breast and thyroid cancer patients. Methods The subjects of this study were 1794 women patients who visited the Ewha Womans University Cancer Center for Women. They included 1119 newly diagnosed primary breast cancer patients and 675 newly diagnosed primary thyroid cancer patients. The patients completed the National Cancer Center Psychological Symptom Inventory (NCC-PSI) during their first follow-up visit after surgery, before starting chemotherapy or radiotherapy. The NCC-PSI is composed of the modified distress thermometer (MDT) and the modified impact thermometer (MIT) for insomnia, anxiety, and depressed mood. Results Anxiety severity was found to be greater in breast cancer patients than in thyroid cancer patients. Significant levels of anxiety, depressed mood and insomnia were present in 28, 24.5, and 20.7% in all the subjects, respectively. Moreover, anxiety symptoms, depressed mood and insomnia interfered with the daily lives of 20, 18.4, and 14.2% of all the subjects, respectively. Dealing with anxiety (18.8%) was found to need the most help, followed by dealing with insomnia (8.9%) and depressed mood (8.7%). Conclusions A significant level of distress was found in about 40% of the total subjects. Nearly 30% of newly diagnosed breast cancer patients reported significant anxiety symptoms and interferences with daily living caused by anxiety, which most commonly needed special care. Early assessment and management of psychological distress, especially anxiety, in breast and thyroid cancer treatment are very important to establish integrated cancer care. |

Early assessment and management of psychological distress, especially anxiety, in breast and thyroid cancer treatment are very important to establish integrated cancer care. 우리나라 국가 암 등록 통계에 따르면 2014년 여성에서 가장 많은 암은 갑상선암으로 유병자 수는 273376명(33.4%)이고, 유방암 유병자 수는 158294명(19.3%)으로 2위이다.1) 암이 처음 발생한 장기를 벗어나지 않은 국한인 경우 갑상선암과 유방암의 5년 상대 생존율은 각각 100.6%, 98.1%이고, 주위 장기나 인접 조직 또는 림프절을 침범한 국소 진행 단계인경우에도 각각 100.4%, 90.6%에 달한다.1) 이처럼 갑상선암과유방암은 예후가 상대적으로 좋은 편이지만, 암의 진단과 치료 과정에서 겪게 되는 심리적 고통 또한 일반인에 비해 발생위험성이 높다.2)3) 암 환자의 정신 건강은 개인의 일상생활뿐아니라 암의 치료와 예후에도 영향을 미치지만, 실제로는 간과되거나 치료를 받지 않는 경우가 많아4)5) 점차 심리적 문제에 대한 평가와 중재의 필요성이 강조되고 있다.6) 암의 치료 기간은 진단 후 완치 판정을 받기까지 일반적으로 5년의 시간이 걸리며 이 기간 중에 신체적 증상, 심리적 고통, 경제적 문제, 사회적 관계의 변화 등 여러 어려움을 겪을수 있는데,7) 이 중 암 환자들이 겪는 가장 흔한 심리적 고통은불안과 우울이다.8) 남성 암 환자보다 여성 암 환자에서 정신질환 발생률이 높으며, 특히 기분장애와 불안장애에서의 발생률이 높다.9) 암 환자가 치료 과정에서 겪을 수 있는 모든 정신적 고통을‘디스트레스(distress)’라고 하며, 암 환자의 20~40%에서 디스트레스가 있고,10) 암 환자들이 가장 필요로 하는 도움은 정신과적 문제에 관한 것이라는 보고도 있다.11) 따라서 미국 암종합네트워크(National Comprehensive Cancer Network)와캐나다 암관리전략협의회(Council of the Canadian Strategy for Cancer Control)에서는 모든 암 환자들에 대한 여섯번째 활력 징후 평가로 디스트레스에 대해 스크리닝하도록권고하였다.12) 우리나라에서도 2009년 국립암센터(National Cancer Center, NCC)에서 ‘암 환자의 삶의 질 향상을 위한디스트레스 관리 권고안’을 개발하여 제공하고 있으며,13) 디스트레스 온도계, 국립암센터 심리증상 평가지(National Cancer Center Psychological Symptom Inventory, 이하 NCCPSI) 등을 통해 디스트레스를 평가하고, 증상에 따라 의료진의 정서적 지지나 약물요법 등을 권고하고 있다.13) 디스트레스에는 ‘겪을 수 있는 모든 정신적인 고통’이라는포괄적인 개념이 담겨 있지만 국내 의료기관의 심리사회적 서비스 자원은 아직까지 한정적이어서 디스트레스를 관리할 수있는 체계가 정립이 되어 있지 않다는 한계점이 있다. 한편 우울, 불안 등의 특정 증상에 대해 평가하는 것이 임상에서 더실제적인 방법이라고 제시하고 있으나 우울이나 불안 증상을평가하는 기존의 척도를 암 환자에게 적용했을 때, 치료의부작용이나 후유증과 중복되는 경우가 많아 신체 증상을 제외한 심리적 증상만으로 평가하는 것을 권고하였다. 따라서디스트레스만 평가하지 않고, NCC-PSI를 사용하여 불면, 불안, 우울 증상에 대해 각각의 디스트레스 정도를 평가하고, 각 증상으로 인해 발생하는 일상생활의 지장을 평가하는 방법이 환자의 심리적 어려움을 더 명확하게 평가할 수 있다.13) 암 치료 초반에 발생할 수 있는 디스트레스를 파악하는 것은 장기적인 치료 과정에서 나타날 수 있는 심리적 고통을 예방하고 치료 예후를 향상시키기 위한 첫 번째 과정이다. 유방암과 갑상선암은 한국 여성에서 가장 흔한 암이며, 비교적 예후가 좋은 편으로 알려져 있다.1) 하지 ...
