다초점인공수정체삽입 후의 굴절력에 대한 수술 전 각막 비구면성의 영향

목적: 이번 연구에서는 각막의 비구면계수와 다초점인공수정체삽입술 후의 굴절예측오차의 상관관계를 통해 술 전 각막 비구면성이술 후 굴절력에 미치는 영향을 분석해보고자 한다. 대상과 방법: 2015년 3월부터 2018년 5월까지 본원에 내원하여 백내장 진단받고 다초점인공수정체삽입술을 시행한 환자 39명(56안)을 대상으로 후향적으로 분석하였다. 다초점인공수정체는 Acrysof IQ RESTOR +2.5D SV25T0 (Alcon Laboratories Inc., Fort Worth, TX, USA), ZEISS AT LISA tri 839MP (Carl Zeiss Meditec AG, Inc., Jena, Germany) 두 가지를 각각 삽입한 환자로 선정하였다. 수술 전 각막의 비구면계수(Q-value)는 rotating Scheimflug camera (Pentacam®, Oculus, Wetzlar, Germany)를 이용하여 6 mm, 7 mm, 8 mm, 9 mm, 10 mm에서 측정하였다. 굴절예측오차(2달 지난 후의 굴절값과 기대굴절값의 차이)와 각막의 6 mm, 7 mm, 8 mm, 9 mm, 10 mm에서 비구면계수 사이의 상관성을 선형회귀분석을 통해 평가하였다. 결과: 두 가지 다초점인공수정체 모두 SRK/T 공식의 굴절예측오차가 수술 전 비구면계수가 6 mm, 7 mm, 8 mm일 때 통계학적으로 유의한 관계를 나타내었다. 비구면계수가 음의 방향으로 갈수록 음의 굴절예측오차가 측정되었으며, 비구면계수가 양의 방향으로 갈수록 양의 굴절예측오차가 측정되었다. 비구면계수와 굴절예측오차의 상관계수는 Hoffer-Q, Haigis 공식보다 SRK/T 공식에서 높게 측정되었다. 결론: 각막의 비구면성은 다초점인공수정체 삽입 시 수술 후 굴절력에 영향을 미치며, 수술 전 비구면계수가 증가할수록 수술 후 원시화된 결과를 유발한다. 특히 SRK/T 공식을 사용했을 경우 이러한 결과를 얻을 수 있었다. Purpose: To analyze the effects of preoperative corneal asphericity on the refractive outcomes of multifocal intraocular lens implantation by correlating Q-values on anterior corneal surfaces and the prediction errors after surgery. Methods: Fifty-six eyes of 39 patients who underwent phacoemulsification, and multifocal intraocular lens implantation were included in this retrospective study. Intraocular lens power was calculated using the SRK/T, Hoffer Q, and Haigis formulas. The Q-values were measured at 6 mm, 7 mm, 8 mm, 9 mm, and 10 mm with a rotating Sheimpflug camera (Pentacam®, Oculus, Wetzlar, Germany). The relationship between the prediction errors and the Q-values at 6 mm, 7 mm, 8 mm, 9 mm, and 10 mm were assessed by linear regression. Results: Two different multifocal intraocular lens models (Acrysof IQ RESTOR +2.5D SV25T0 [Alcon Laboratories Inc., Fort Worth, TX, USA] and ZEISS AT LISA tri 839MP [Carl Zeiss Meditec AG, Inc., Jena, Germany]) were implanted in 56 eyes. In both groups, regression analyses showed statistically significant relationships between the prediction errors and the Q-values at 6 mm, 7 mm, and 8 mm using the SRK/T formula. The correlation coefficient between the Q-values and the prediction errors were higher using the SRK/T formula than using the Hoffer-Q and Haigis formulas. Conclusions: Corneal asphericity affected refractive power during implantation of a multifocal intraocular lens and, as the preoperative Q-value increased, it resulted in a hyperopic refractive outcome for oblate corneas. These results were especially prominent when the SRK/T formula was used. KCI Citation Count: 0
