Abstract Infected walled-off pancreatic necrosis (WOPN) is dangerous complication of acute pancreatitis. Open necrosectomy and post-operative irrigation of necrotic cavity have been conventional treatment for infected pancreatic necrosis, however, recently minimally invasive techniques such as endoscopic necrosectomy has been regarded as preferred treatment method for infected WOPN. Endoscopic necrosectomy provides a targeted approach with a reduction in the systemic inflammatory response and avoidance of wound complications. Non alcohol related Wernicke encephalopathy is rare disease caused by thiamine deficiency due to intravenous feeding, and presented an encephalopathy, oculomotor dysfunction, gait ataxia. We report herein a case of Wernicke encephalopathy after successful endoscopic necrosectomy for infected WOPN.

중증 급성 췌장염의 경우 국소 합병증으로 구역성 췌장괴사(walled-off pancreatic necrosis)가 발생하면 보존적 치료만으로 호전을 기대하기 힘들기 대문에 수술로 배액이나 괴사제거술을 시행하는 것이 고전적인 치료법이었다. 최근 치료 내시경의 발전으로 인하여 내시경적 치료로 대체되고 있으며 환자의 임상양상, 보존적 치료에 대한 치료 반응, 해부학적 괴사 위치 등을 고려하여 선택되어야 한다. 저자들은수술적 치료가 불가한 중증 급성 괴사성 췌장염이 진단된 환자에서 반복적인 내시경 괴사제거술을 성공적으로 시행하고, 치료 도중에 베르니케뇌병(Wernicke encephalopathy)이발생하여 티아민을 투여하고 성공적으로 치료한 1예를 경험하여 문헌고찰과 함께 보고하는 바이다.
