율곡의 공부론을 통한 의학교육에서의 프로페셔널리즘 가치에 대한 고찰

이 글의 목적은 의료진이 인간 존재에 대한 철학적 인식 없이 객관적인 질병만을 치료의 대상으로 여겨 일어나는 문제에 관하여, 율곡의 시선으로 의학교육이 나아가야 할 방향성을 제안하는 것이다. 최상위권 수험생들의 의과대학 쏠림이 사회적인 문제로까지 이야기되는 한국 사회에서, 의학에서의 프로페셔널리즘 역량은 의료행위가 단순히 치료의 행위에 머무르지 않고, 딜레마적 상황에서도 공익적 가치를 창출하고, 선도해 나아가는 사회적인 영향력까지 실천해 내는 능력이다. 바람직한 의사 양성을 위한 의학교육은 빠른 사회적 변화 아래에서 지식, 술기 외에도, 사람, 사회, 질병에 대한 통합적 이해로 임상 현장에서 사회, 환자, 보호자에게 신뢰를 주는 의대생을 길러낼 수 있어야 한다. 이것은 사사로움을 앞세우지 않고, 백성을 위한 헌책에 온 정성을 다하는 것을 일상에서 역행(力行)으로 보여 준 율곡의 실천적 삶과 같은 의미의 맥락을 가진다. 이를 위한 구체적인 실천사항인 (1)의사는 환자의 가치관을 편견과 차별 없이 존중하고, 자신의 신념체계만을 강요하지 않는다 (2) 의사는 직업상 발생할 수 있는 이해 상충 상황을 이해하고 적절하게 관리한다 (3)의사는 최선의 진료를 위해 자율성을 가져야 한다는 세 갈래로 나누어 살펴보고자 한다. 즉, 의료 전문가 정신(professional identity)의 함양을 위해, 율곡의 공부론 중 입지(立志), 궁리(窮理), 거경(居敬)의 함의가 프로페셔널리즘 양성의 기본이 될 수 있다는 것을 드러낸다. 의료행위는 질병에 대한 치료에 국한되는 것을 넘어서, 환자에게 치료 행위가 미칠 수 있는 여러 요소에 대한 이해가 중요하다. 그래서 의료 프로페셔널리즘에 대한 율곡의 공부론적 접근은 현시점에서 의학교육이 어떤 교육철학적 토대 위에 세워져야 하는지 성찰하게 할 계기를 줄 것이다. In other words, Yulgok's unity of knowledge and conduct, which we should pay attention to for the cultivation of medical professionalism, emphasizes the meaning of a practical life centered on positioning, researching, and giggling. If this is applied to doctors as professionals, it can be emphasized that medical practice is not limited to the treatment of diseases, but it is important to understand the various factors that can affect patients.
