LASIK 후 나안 시력이 20/40 이하인 원인

목적 : 라식(laser in situ keratomileusis : LASIK) 수술 후 나안시력이 20/40 이하인 환자들의 원인에 대해 알아보았다. 대상과 방법 : 1997년 7월부터 1999년 12월까지 본원에서 라식을 시행한 -6.0 D 이상의 근시 환자 중 6개월 이상 후향적 조사 시 경과관찰이 가능하였던 289안을 대상으로 하여 수술 후 나안시력과 굴절상태, 각막지형도, 각막두께 등을 조사하였다. 그 중 9안은 재수술을 시행하였다. 결과 : 289안 중 51안(17.64%)이 나안시력이 20/40 이하였다. 그 원인으로는 근시로의 이행(22/51), 중심이탈(10/51), 근시의 부족교정(5/51), 각막돌출(5/51), 난시의 부족교정(4/51), 중심부 융기(2/51), 수술 중 각막절편 이탈(1/51), 수술 후 각막절편 이탈(1/51), 불완전 각막절제(1/51)이었다. 결론 : 라식 수술은 근시 교정을 위한 안전하고 효과적인 방법이다. 그러나 본 연구에서 볼 수 있듯이 17.64%나 되는 많은 환자들이 20/40 이하의 나안 시력을 얻었다. 라식 수술 후 좋은 나안 시력을 위해 위의 여러 원인 중 교정할 수 있는 요인을 잘 조정해 할 것이다. <한안지 42(11):1549-1554,2001> Purpose : To evaluate the causes of low uncorrected visual acuity below 20/40 after LASIK (laser in situ keratomileusis). Methods : 289 eyes (more -6.0 D myopia) that had LASIK between July 1997 and December 1999 by one surgeon were observed for at least 6 months. We evaluated their uncorrected visual acuity and ocular state. Results : 51 eyes (17.64%) of 289 eyes have lower uncorrected visual acuity than 20/40 after LASIK. The causes were myopic regression (22/51), decentering (10/51), undercorrection of myopia (5/51), corneal ectasia (5/51), undercorrection of astigmatism (4/51), central island (2/51), free flap (1/51), postoperative flap displacement (1/51) and incomplete keratome pass (1/51). Conclusion : Although LASIK is a safe and effective procedure, many patients had lower uncorrected visual acuity less than 20/40 after LASIK.
