Abstract The aim of this study was to investigate efficacy of posterior decompression with segmental fusion at a maximum cord compression level (sPDF) for the K-line (+/-/+) OPLL cases. This study enrolled 29 patients with K-line (+/-/+) OPLL who underwent laminoplasty (LMP) or sPDF. These patients were included in propensity score calculation and 1-to-1 matching resulted in 7 pairs of LMP and sPDF cases. The JOA recovery rate and the acquired score were significantly higher in sPDF group. For the K-line (+/-/+) OPLL case, sPDF seems to be a useful operative method.
