Deconditioningに対する理学療法が末梢血管反応に与える変化

The purpose of this study was to examine the influence of physical therapy on peripheral vascular response in a bed-rest deconditioned patient. Subject was one patient with idiopathic interstitial pneumonia. He received our program after bed rest (two periods). Our program included muscle strength exercise, sitting (3-4 hours a day) and gait. The measurements of periflux blood flow and cardiorespiratory response under sitting and standing position were performed every two weeks. Peripheral blood flow in the fingertip was measured by Laser Doppler flowmeter. Blood flow response pattern, pulse rate, blood pressure and RPE (rating of perceived exertion) were measured in a 5-min sitting position and in a 5-min standing position. Peripheral vascular response, control of blood pressure and physical endurance recovered in 2-6 weeks. The results suggest that our program may influence on peripheral vascular response and be useful to recover from deconditioning. 今回，我々は臥床後にDeconditioningを呈した特発性間質性肺炎患者1症例に理学療法を行うとともにレーザードップラー法により皮膚血流量を測定する機会を得た。我々は，症例の2度の臥床期間（3ヵ月，1ヵ月）後，原疾患が安定した時期に，理学療法（筋力トレーニング，座位，立位，歩行）を実施した。そして，理学療法の開始時と2週間毎に座位，立位負荷試験（座位5分間，立位5分間）を行い，皮膚血流量，心拍数，血圧，SpO2，Borg指数を測定した。結果，臥床後に低下していた皮膚血流反応，血圧調節機能および運動耐容能は2〜6週後に改善した。この結果より，本症例において，理学療法が末梢血管反応に変化を与え，Deconditioningの改善に影響したことが示唆される。
