Epicardial ablation for ventricular tachycardia related to dilated phase hypertrophic cardiomyopathy
症例は拡張相肥大型心筋症の59歳,男性.2001年,持続性の心室頻拍(VT)に対し植込み型除細動器(ICD)植え込み施行.2010年11月,2011年2月にICD頻回作動のため,心臓電気生理学的検査(EPS)/アブレーションを施行.右室流出路および左室心基部に低電位部位を認め,遅延電位およびペースマップを指標にアブレーション施行した.しかしながらEPS中に誘発されなかった,頻拍周期が長く(530ms)QRSの立ち上がりの遅い右脚ブロック型下方軸のclinical VTが再発したため,2011年2月に2回目のアブレーションを施行した.心窩部より心外膜穿刺を行い,洞調律下に心外膜マッピングを施行....

Abstract | 症例は拡張相肥大型心筋症の59歳,男性.2001年,持続性の心室頻拍(VT)に対し植込み型除細動器(ICD)植え込み施行.2010年11月,2011年2月にICD頻回作動のため,心臓電気生理学的検査(EPS)/アブレーションを施行.右室流出路および左室心基部に低電位部位を認め,遅延電位およびペースマップを指標にアブレーション施行した.しかしながらEPS中に誘発されなかった,頻拍周期が長く(530ms)QRSの立ち上がりの遅い右脚ブロック型下方軸のclinical VTが再発したため,2011年2月に2回目のアブレーションを施行した.心窩部より心外膜穿刺を行い,洞調律下に心外膜マッピングを施行.心外膜側右室および左室前壁,心基部に低電位部位を認めた.心内膜側のペーシングでは誘発されなかったclinical VTが心外膜側からの刺激により容易に誘発された.VT中に心内膜および心外膜側両方からマッピングを行い,心外膜側前壁基部の低電位部位に長い拡張期電位を認め,同部位でのペーシングにてentrainment with concealed fusionを認め,post pacing intervalも頻拍周期に一致した.同部位への通電によりVTは4秒で停止し,誘発不能となった.近傍の遅延電位に通電を追加し,終了した.以後,VTの再発を認めず,経過良好である. |
