Abstract | Objective: To evaluate the usefulness of ultrasonography of the carotid artery as an atherosclerosis screening method, we investigated the association between serial changes in carotid lesions and risk factors for atherosclerosis. Methods: The subjects were 66 cases in whom carotid lesions could be followed up for 5 years using ultrasonography in Takeda Hospital and affiliated hospitals. In these lesions, intima-media complex thickness (IMT)and the plaque score were investigated. Results: The mean follow-up period was 1,835.9±110.1 days. After follow-up, IMT and the plaque score significantly increased to 0.12±0.16mm (p<0.0001) and 0.87±1.67 (p<0.0001), respectively. With respect to the association with risk factors for atherosclerosis, these parameters were slightly higher in the group with risk factors, however, there were no significant differences. Concerning plaque calcification, these parameters were higher in the group with calcification. In particular, these parameters were more markedly increased in the group in which calcification was newly detected, and the plaque score was significantly increased (p =0.0002). Conclusions These results suggest the usefulness of investigating IMT, plaque, and plaque calcification by ultrasonography of the carotid artery in atherosclerosis screening. |

Conclusions These results suggest the usefulness of investigating IMT, plaque, and plaque calcification by ultrasonography of the carotid artery in atherosclerosis screening. 目的:頸動脈超音波検査の動脈硬化スクリーニングとしての有用性について,頸動脈病変の経年変化に注目し,動脈硬化危険因子との関連とともに検討を行った.方法:武田病院および関連施設において,超音波法を用いて,頸動脈病変について5年の経過をおいて観察が可能であった66例を対象とした.頸動脈病変は,内膜中膜複合体厚(intima-media complex thickness:IMT),プラークスコアについて検討を行った.結果:観察期間は1,835.9±110.1日であった.観察前後で,IMTは0.12±0.16mm(p<0.0001),プラークスコアは0.87±1.67(p<0.0001)の有意な増加が見られた.動脈硬化危険因子との関連については,危険因子を有する群で増加傾向を示したが,統計学的には有意差を認めなかった.プラークの石灰化病変に注目した検討では,石灰化が存在する群で増加傾向を認めた.特に,新たに石灰化を認めた群では,より強い増加傾向を認め,プラークスコアは有意に増加した(p=0.0002).結論:頸動脈超音波検査によるIMT,プラークの観察およびその石灰化病変の観察が,動脈硬化スクリーニングとしての指標となることが示唆された. |
