An 86-year old man was admitted to our hospital for dyspnea and lower abdominal pain. Abdominal enhanced CT scan showed ACF. However, the patient died while we were preparing for surgery. It is important to diagnose ACF quickly and certainly before cardiopulmonary arrest because it is associated with a low survival rate. ACF should be considered if patients have symptoms of heart failure with AAA, a pulsating mass in the abdomen, or continuous abdominal murmur.

ACF should be considered if patients have symptoms of heart failure with AAA, a pulsating mass in the abdomen, or continuous abdominal murmur. 【緒言】稀な腹部大動脈瘤-下大静脈瘻の1例を経験したため報告する。【症例】86歳の男性。呼吸困難と下腹部痛を主訴に当院へ救急搬送された。搬送2日前に意識消失発作を起こし他院へ救急搬送された際は異常を指摘されていなかった。体幹部造影CT検査で腹部大動脈と下大静脈のいずれにも造影効果を認め, 腹部大動脈瘤の下大静脈との瘻孔と診断した。緊急手術の準備中に心停止となり, 心肺蘇生術に反応なく死亡した。【考察と結語】本疾患は経過中にショックや心停止となることがある。特に, 術前に心停止を呈した症例の救命率は低く, 本疾患は心停止に陥る前に迅速かつ確実に診断・治療することが重要である。そのためには突然の意識消失発作や腹部大動脈瘤を伴う原因不明の心不全, 腹部の拍動性腫瘤や血管の連続性雑音を呈した場合には, 稀ながら本疾患も念頭に置く必要がある。
