潰瘍性大腸炎に限局性筋膜炎を合併した1例
症例は36歳男性．4年前に潰瘍性大腸炎と診断．2020年10月に血便を来し，潰瘍性大腸炎再燃との診断で，サラゾスルファピリジン内服開始15日後に右下腿痛と腫脹が出現した．MRIで右下腿の筋膜や血管周囲にT2脂肪抑制画像で高信号，造影T1脂肪抑制画像で増強効果を認めた．右下腿の腫脹は自然軽快するも，左下腿にも腫脹，疼痛が出現した．MRIでは右と同所見で，筋生検では筋組織に異常所見は認めず，臨床経過から限局性筋膜炎と診断した．Crohn病に合併して限局性筋膜炎を来すgastrocnemius myalgia syndromeの報告が多いが，潰瘍性大腸炎では稀である．状況に応じて安静及びステロイドな...
