特発性脊髄ヘルニアの修復術後に腹側硬膜壁内囊胞が拡大し脊髄症を生じた1例

Abstract はじめに：特発性脊髄ヘルニアidiopathic spinal cord herniation（ISCH）の術後に，これまで報告のない特異な様式で髄液が貯留した症例を経験した．症例：57歳男性．Brown-Séquard症候群で発症．T4/5高位のISCHを腹側硬膜裂孔から続く壁内囊胞から解除し，expanded polytetrafluoroethylene（ePTFE）人工硬膜によるハンモック法を用い修復した．術後7ヶ月に症状が再発し，MRIでは頚椎から腰椎の硬膜管腹側に液体信号がみられ髄液漏と診断，再手術を施行した．癒着膜で覆われたePTFE人工硬膜を除去すると硬膜裂孔の腹側に広大な腔がみられた．視診と同腔の造影所見から，既存の腹側硬膜の壁内囊胞が頚椎と腰椎まで拡大したものと判明した．硬膜裂孔を拡大し手術を終えた．結語：本症例では，ISCHの修復に用いたePTFE人工硬膜による癒着が一方向弁となり，壁内囊胞に髄液を浸透させ，ここから腹側硬膜を脊柱管のほぼ全長に渡り分離・拡大させたと考えられた．
