頚椎神経根症状を主訴とした類骨骨腫の一手術例
類骨骨腫(Osteoid osteoma)は20歳以下の若年男性に多く発生し,好発部位は下肢などの長管骨である.大きさは15 mm以上になることはないとされている.症状発生にはプロスタグランジンの関与があり,小さい骨腫ではあるが,周囲組織に浮腫をもたらし,疼痛を惹起している.症例は17歳の女性で,頚椎C5椎弓根より発生し,進展して頚部痛とC6神経根症状を呈した.治療法としてCT,MRIをガイドとしたablationでは低侵襲ではあるが,周囲の重要組織への影響が危惧される.本症例のような神経組織近傍に発生した脊椎病変では,CTガイドを利用したopen surgeryがより安全で正確にnidus摘...
