Abstract 「はじめに」内頚動脈系の非分岐部動脈瘤のなかにはblister-like aneurysmと呼ばれるneckのきわめて脆弱な動脈瘤が存在し1)6)12-14), これは術中操作の際に出血をきたしやすいことやクリッピング困難なことが知られている. 一方で少数ながら健常なneckが存在しクリッピング可能な例もあること13)19)や, 親動脈の一部ごとクリップすることでクリッピング可能な症例も存在する. われわれは過去5年間で5例の内頚動脈系の非分岐部動脈瘤の治療を経験した. これらの治療結果からこの部の動脈瘤の診断治療について検討したので報告する. 「対象」2002-2007年の5年間に経験した内頚動脈系の非分岐部に発生し, 治療を行った脳動脈瘤5例が対象である. 内頚動脈が4例, 前大脳動脈A1部1例で年齢は32-73歳(平均57.8歳), 男女比は3：2であった. 発症形態は3例がくも膜下出血であり1例は他部位の破裂動脈瘤に合併した未破裂例であり, A1部の動脈瘤は脳内出血を伴ったくも膜下出血であった.
