腹腔鏡下前立腺全摘除術：10年間の治療成績

我々は，限局性前立腺癌に対して2000年より腹腔鏡下前立腺全摘除術を開始し，2009年までの10年間で330例に施行した．手術時年齢は中央値65.5歳，術前PSAは中央値7.6ng/mlで，術後観察期間は中央値51か月であった．手術時間は中央値284分，出血量は中央値400mlで，開放手術への移行は4例であった．術中合併症は22例（6.7%）に生じ，直腸損傷は6例（1.8％）に認めた．切除断端陽性は114例（34.5%），5年PSA非再発率は76.7％で，多変量解析では被膜外浸潤と切除断端陽性が危険因子となった．中長期的な癌制御においては開放手術と遜色ない結果であったが，切除断端陽性率はやや高...
