成人臍ヘルニア手術の7例
成人臍ヘルニアは本邦では比較的まれな疾患とされている。今回われわれは, 嵌頓4例を含む7例の手術症例を経験したので報告する。年齢は49歳から79歳で男性2人, 女性5人であった。Body Mass Index (BMI) では4例が肥満ないしは高度肥満症例であった。嵌頓例の2例が緊急手術となりうち1例に腸切除が必要であったが, それ以外は待機手術が可能であった。全例術後合併症なく経過し退院した。成人臍ヘルニアは腹水貯留や多産, 高度肥満などによる腹圧上昇が成因で, 小児臍ヘルニアと異なり嵌頓, 絞扼の危険性が高いと言われている。また高度肥満や肝硬変症例では周術期合併症が多いとも言われており,...

We recently experienced seven surgical cases of umbilical hernia, including four incarcerations. The ages of the patients ranged from 49 to 79 years ; two were men and five were women. As shown by their body mass index (BMI) values, four cases were either obese or severely obese. Two cases with incarceration required emergency surgery, and one of these patients required an intestinal resection ; delayed surgery was possible in the remaining cases. After their surgeries, all the patients were free from complications and were discharged from the hospital uneventfully. Adult umbilical hernia is principally caused by a higher abdominal pressure resulting from the storage of ascites, pregnancy, or severe obesity. Unlike childhood umbilical hernias, the possibility of incarceration and strangulation is thought to be relatively high in adult cases. Post-surgical complications are reportedly common in patients with hernias caused by obesity or cirrhosis of the liver.
