| Abstract | 「I 緒言」 筋萎縮性側索硬化症(Amyotrophic Lateral Sclerosis ; ALSと略す)は運動神経が選択的に侵される進行性の神経変性疾患であり, 原因と治療法は解明されていない. 発病後, 全身の筋萎縮と筋力低下を来たし, 独立歩行不能, 発語不能, 人工栄養導入, 上肢機能廃絶, 気管切開施行の状態を順次余儀なくされ, 侵襲的人工呼吸器(Tracheostomy, Invasive Positive Pressure Ventilation ; TPPVと略す)を装着しなければ数年で死に至る. 発病後, 様々な身体的苦痛や社会的困難, 精神的苦悩が生じ, 一旦装着すれば外せないTPPV選択の葛藤など過酷な苦悩を経験することが多数報告されてきた. さらに, 複数の身体機能障害が重度に至ったTPPV装着後は, 精神的・霊的, 身体的および社会的な多岐に及ぶ困難を抱えるとの報告がある. 一方, 困難と向き合いより良い生活を目指すために, 良好な人間関係や楽しみなどの対処資源をもつ重要性も明らかになりつつある. |
