術前化学療法中のG-CSF製剤投与により大動脈炎を発症した乳癌の1例

| Abstract | 癌化学療法において，発熱性好中球減少症発症リスクが高い患者に対してG-CSF製剤の予防的投与が推奨されるが，一方で近年，G-CSF製剤使用後の大動脈炎の合併リスクも0.47-2.7％程度に認めるとの報告もあり，重要視されている．われわれは，長期作用型G-CSF製剤pegfilgrastimの複数回投与後に大動脈炎を発症した症例を経験したので報告する．症例は53歳，女性．右乳癌に対しpegfilgrastimを併用した術前化学療法を施行した．Dose-dense（dd）EC療法4コース，dd-paclitaxel療法1コースを有害事象なく完遂したが，2コース目のpegfilgrastim投与後から発熱と右下腹部痛を認めた．2週間後にも改善を認めず炎症反応の上昇，CTにて大動脈周囲の炎症像を認め，G-CSF製剤による薬剤誘発性血管炎が疑われた．ステロイドは使用せず解熱鎮痛薬の投与のみで改善を認めた．この時点で乳癌は完全奏効が得られたため，術前化学療法は中止し手術を施行した．その後は症状の再燃を認めず，術後42日目のCTで大動脈炎の所見は消失していた．G-CSF製剤初回投与時に異常を認めなくても複数回目で薬剤誘発性大動脈炎が発症する可能性があることに留意する必要がある． |
